Here is a relatively simple 5 band equalizer that uses an IC LA3600 especially made for 5 band equalizer application. It has a relatively simple circuit that is easy for PCB layout. LA3600 make it easy to build your own 5 band equalizer with relatively few components. It will also be easier to design a single PCB since it only have very few components.

The frequency of each controls can be change for your preference. Below is the formula for computing the frequency.
PCB Layout
PCB layout is done using eagle PCB. It is only a single layer PCB so it will be easy to build for DIY. You can download the design file below and free to modify it for your own preference. You can further decrease the size of the PCB if you want.
